Be prepared for your next appointment!
Bring your BC Services Card
If you have a BC Services Card, please bring the card to your appointment.
Bring your Glasses Prescription
If you wear prescription glasses, bring them to your appointment. If you wear contact lenses, have the brand and type of lenses ready. (Eg. Acuvue Oasys 1 day)
Bring your Insurance Information
If you would like us to directly bill your eye exam to your insurance, we require the plan or policy number and member ID. This may be on a card provided by your insurance, or can be found on your insurance's app.
Running late?
Please arrive on time or let us know if you will be running late. If you’d like to reschedule or cancel your appointment please let us know as soon as possible.
